Mobile Health Provider (APRN or PA) Bring Healthcare to the Community - Not the Other Way Around Greater Seacoast Community Health (GSCH) Full-Time | Exempt $121,680 - $132,600 annually (based on years of experience) Care Without Walls. At Greater Seacoast Community Health, our Mobile Health Program is redefining access to care-bringing high-quality, compassionate services directly into the community. We are seeking a Mobile Health Provider (APRN or Physician Associate) who thrives in dynamic environments and is passionate about meeting patients where they are. This is more than a clinical role. It's a chance to remove barriers, expand access, and change lives-one patient at a time. Why This Role Matters As a Mobile Health Provider, you will: Deliver primary and preventative care outside traditional settings Support underserved populations with accessible, patient-centered care Collaborate across a multidisciplinary care team Play a key role in advancing health equity in the Seacoast community Your work directly impacts patients who may otherwise go without care. What You'll Do Provide comprehensive primary care, including preventative exams, diagnosis, treatment, and follow-up Coordinate care with internal teams and community-based resources (behavioral health, social services, specialty care, etc.) Document all patient care accurately and timely in the EHR system Participate in case conferences, provider meetings, and quality improvement initiatives Support and mentor students (APRN/PA) when applicable Contribute to the development and refinement of clinical protocols Deliver care with professionalism, compassion, and respect What Makes This Role Unique Mobile care delivery - not tied to a traditional clinic Strong team-based, integrated care model Opportunity to work in a flexible, community-focused environment ❤️ Direct impact on access to care and population health
What You Bring Required Qualifications:
APRN:
Current
NH RN & APRN
license Board Certified (ANCC or AANP)
OR Physician Associate:
