We are seeking a skilled and compassionate Nurse Practitioner to provide comprehensive primary and palliative care services while improving quality of life and supporting patients and families through complex healthcare journeys. Position Overview The Primary & Palliative Care Nurse Practitioner (NP) is responsible for providing comprehensive medical care to patients with acute, chronic, and serious illnesses. This role focuses on disease management, symptom control, advance care planning, and reducing avoidable hospitalizations while promoting patient comfort, dignity, and quality of life. The patient population includes residents in skilled nursing facilities (SNFs), long-term care facilities, and assisted living facilities. The ideal candidate is compassionate, clinically skilled, and experienced in managing medically complex patients while collaborating with interdisciplinary teams to provide holistic, goal-concordant care. Essential Functions Provide primary and palliative care services to patients in post-acute, long-term care, assisted living, and community-based settings Perform comprehensive history and physical examinations for acute, chronic, and medically complex patients Assess, diagnose, and manage a wide range of medical conditions and chronic diseases Develop individualized care plans that align with patient goals, values, and clinical needs Lead and document goals-of-care discussions, advance care planning, and code status conversations Facilitate completion and updates of advance directives, POLST forms, and DNR/DNI orders Manage pain and complex symptoms, including dyspnea, nausea, anxiety, depression, agitation, and insomnia Prescribe and manage medications in accordance with state and federal regulations Review and interpret laboratory, diagnostic, and imaging results and implement appropriate interventions Evaluate new admissions and post-hospital discharges, including medication reconciliation and care plan development Identify patients who may benefit from hospice services and facilitate timely referrals when appropriate Participate in readmission reviews and implement strategies to reduce avoidable hospitalizations Collaborate with physicians, nursing staff, social workers, therapists, hospice providers, and interdisciplinary teams Educate patients, families, and caregivers regarding diagnoses, treatment options, prognosis, and care planning Document all patient encounters and clinical decisions accurately within the electronic medical record (EMR) Maintain compliance with HIPAA, regulatory requirements, and organizational policies Education & Experience Master's or Doctorate degree in Nursing (MSN or DNP) from an accredited program Current and unrestricted Nurse Practitioner license Current DEA registration and prescriptive authority Experience in primary care, geriatrics, palliative care, hospice, family practice, internal medicine, or post-acute care preferred, but not required; training opportunities available for qualified candidates Preferred Skills & Competencies Strong clinical assessment and diagnostic skills Experience managing medically complex and chronically ill patients Knowledge of symptom management and palliative care principles Excellent communication and interpersonal skills Ability to conduct difficult conversations regarding goals of care and advance care planning Strong organizational and time-management skills Ability to work independently while collaborating effectively with interdisciplinary teams Experience with EMR systems and telehealth platforms preferred Why Join Anpex Health?
