Qualifications:
Education:
Master's degree in Nursing from an accredited program.
Licensure:
Must have an active New Jersey State Nurse Practitioner license
Experience:
New graduates are welcome to apply.
Technical Skills:
Proficient in performing comprehensive geriatric assessments, medication management, wound care, chronic disease management, and utilizing eClinicalWorks electronic health records.
Soft Skills:
Strong clinical judgment, excellent communication and interpersonal skills, empathy, patience, and the ability to work collaboratively with nursing home staff, families, and physicians.
Key Responsibilities:
Provide comprehensive primary care services to nursing home residents, including physical examinations, health assessments, and preventive care. Diagnose and manage acute and chronic medical conditions commonly seen in the elderly, such as hypertension, diabetes, heart failure, dementia, infections, and respiratory illnesses. Prescribe medications, monitor for adverse drug reactions and polypharmacy, and adjust treatment plans as needed. Order, interpret, and analyze diagnostic tests and laboratory results. Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team (nurses, social workers, rehabilitation therapists, and attending physicians). Monitor residents' health status through regular rounds, assessments, and communication with nursing staff. Educate residents and families on disease management, medication adherence, treatment options, and end-of-life care when appropriate. Respond to acute changes in resident condition and coordinate timely interventions or transfers to higher levels of care. Maintain accurate, thorough, and timely documentation in electronic medical records in accordance with nursing home regulations.
