This is an opportunity to make a real impact on patients' lives while working with a supportive and mission-driven team.
Responsibilities:
Provide a full scope of primary care, including new admissions, routine follow-up visits, and comprehensive medical management for residents Collaborate with an interdisciplinary team to ensure high-quality, patient-centered care
Requirements:
Active Nurse Practitioner license in the state of NJ Minimum of 6 months NP experience Prior nursing home or long-term care experience highly preferred
Schedule:
Full time - Monday till
Friday Compensation & Benefits:
