Position Overview We are seeking a skilled and compassionate Primary Care Nurse Practitioner (NP) to join our Mobile Care team. This full-time role involves traveling to Assisted Living Facilities in and around Princeton, NJ and surrounding areas to manage chronic and acute conditions and build meaningful relationships with patients and their families. You'll work independently with flexible scheduling, supported by a collaborative clinical team. Key Responsibilities Conduct primary care visits in Assisted Living Facilities Perform physical exams, diagnostic assessments, and routine screenings Manage chronic conditions and acute illnesses Prescribe medications and develop individualized care plans Coordinate with other healthcare providers and community resources Document visits and maintain accurate medical records Educate patients and caregivers on health management and preventive care Communicate with facility staff, patients, and families regarding car Licensed Nurse Practitioner in the state of New Jersey Board-certified in Family Medicine or Adult-Gerontology Minimum of 1 year of clinical experience (geriatrics preferred) Strong diagnostic and clinical decision-making skills Excellent communication and interpersonal abilities Valid driver's license and reliable transportation What We Offer Competitive hourly rate ($80-$85/hour based on experience) Flexible scheduling to support work-life balance Autonomy in clinical practice with supportive oversight Benefits Health insurance (medical, dental, and vision) 401(k) Paid time off (PTO) and Sick Time
