Compensation:
$75.00 - $82.00/hour About the Role NJ Palliative Care Solutions is seeking a compassionate and skilled Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our growing team. This is a fully in-person role providing care across hospital and post-acute settings throughout New Jersey. We proudly partner with leading healthcare organizations, including RWJ Barnabas Health and Complete Care , delivering high-quality, patient-centered palliative care. Key Responsibilities Provide comprehensive palliative care in hospital and post-acute environments Perform patient assessments, develop individualized care plans, and manage ongoing treatment Educate and support patients and families regarding goals of care and treatment options Collaborate with physicians, facility staff, and interdisciplinary teams to ensure continuity of care Maintain accurate and timely clinical documentation in compliance with regulatory standards Qualifications Active, unrestricted New Jersey NP or PA license (required) Experience in palliative care, geriatrics, hospice, chronic disease or related fields preferred Strong clinical judgment, communication, and interpersonal skills Commitment to compassionate, patient-centered care Why Join Us?
