We are committed to educating consumers and serving patients and families with the utmost dignity and grace.​ Role Overview As a Registered Nurse (RN) at Grace Healthcare Services, you will be an integral member of our Interdisciplinary Team (IDT), responsible for assessing and addressing the comprehensive needs of patients and their families. Your role involves initiating appropriate interventions upon admission and ensuring a continuous, responsive plan of care throughout the patient's journey.​
Key Responsibilities:
Patient Care Follow the plan of care of patient/family care and services. Outlines admission plan of care with the IDT and RN Case Manager. Provides nursing care as determined by the plan of care and the RN Case Manager. Educates patient/family and other primary caregivers, in appropriate care techniques. Serves as the patient/family advocate by maintaining regular communication with ALF staff, long term care facility staff, and RN Manager. May participate in providing bereavement support to families and associated bereavement activities as appropriate. Ensure and document appropriate use of medication, durable medical equipment, and supplies. Participates in team meeting. Provides accurate documentation with visit itineraries to the office. Documentation must be completed according to hospice, state, and federal guidelines/standards. Documentation must be prepared and turned in according to these guidelines. Follows RN care plan appropriately/reports to RN case manager on each patient. Monitors the recertification/reauthorization process and provides input for this process. Reports necessary information to the Clinical Director/Regional Manager/RN case manager and other members of the IDT for on-call shift report. Responds to on-call issues as clinically appropriate. Works under the direction of the RN case manager. Quality Improvement Integrates quality improvement results into the established plan of care. Participates in quality assessment and improvement activities as assigned by the Regional Manager/RN case manager or QAP1 Manager. Participates in proactive patient calls and visits. Participates in substantive chart reviews as assigned by the Regional Manager Professional Development Participates in the orientation of new team members as assigned by the RN Case Manager. Attends regularly scheduled in-service and educational conferences. Develops and achieves professional growth goals and objectives and reviews these with his/her supervisor on a regular basis. Represents Grace Healthcare Services in a positive manner. Participates in educational activities with Grace Healthcare Services as well as outside the Grace Healthcare Services program. Ensures that Grace Healthcare Services' best practices are incorporated into all job activities/responsibilities. Customer Service Promotes a customer service-oriented approach to care & customer relationships. Participates in customer conflict resolution. Access to reliable transportation. Adhere to the practice of confidentiality regarding patients, families, staff, and the organization. Qualifications:
Currently licensed to practice nursing in the state A minimum of two years nursing experience in acute-care hospital nursing in either medical, surgical or oncology unit or other commensurate experience. Communication Skills
