Nurse Practitioner (NP)
Job Overview:
We are looking for a compassionate and experienced Nurse Practitioner to provide primary and ongoing care to residents in a skilled nursing facility in Newark, NJ. The NP will work closely with physicians, nurses, and the interdisciplinary team to ensure residents receive high-quality medical care.
Responsibilities:
Conduct routine and acute patient assessments for nursing home residents Manage chronic conditions and develop treatment plans Respond to changes in residents' conditions and provide timely care Order and interpret diagnostic tests and labs Prescribe medications in accordance with New Jersey regulations Coordinate care with physicians, nursing staff, and families Ensure proper documentation in the EMR Participate in care plan meetings and regulatory compliance
Requirements:
Active New Jersey Nurse Practitioner license Board Certified Nurse Practitioner (FNP, AGNP, or GNP preferred) Experience in skilled nursing facility (SNF), long-term care, or geriatrics preferred Strong clinical decision-making and communication skills Ability to work collaboratively with nursing home staff
