Job Description

Job Overview Wound Care NP/PA We are seeking a dedicated and skilled Wound Care Nurse Practitioner to join our healthcare team. In this role, you will be responsible for assessing, diagnosing, and treating patients with various types of wounds. You will work closely with a multidisciplinary team to develop comprehensive care plans that promote healing and enhance the quality of life for our patients. The ideal candidate will possess strong clinical skills, excellent communication abilities, and a passion for patient-centered care. Responsibilities Conduct thorough assessments of patients with acute and chronic wounds. Develop individualized treatment plans based on patient needs and evidence-based practices. Perform wound care procedures, including dressing changes, debridement, and advanced wound therapies. Collaborate with physicians, nurses, and other healthcare professionals to coordinate patient care. Educate patients and their families on wound care management and prevention strategies. Monitor patient progress and adjust treatment plans as necessary. Maintain accurate documentation of patient assessments, treatments, and outcomes. Participate in case management activities to ensure continuity of care. Stay current with advancements in wound care techniques and technologies. Requirements Master's degree in Nursing with specialization as a Nurse Practitioner. Current Nurse Practitioner certification and state licensure to practice. Experience in wound care or related fields is preferred. Strong skills in case management and patient education. Basic Life Support (BLS) certification required.
