Job Summary Job description: Medicare Advantage Care Partners is seeking a Nurse Practitioner (NP) to join our Medicare Advantage-focused primary care team in Carson City, NV!

Pay:

$120,000.00 - $150,000.00 per year plus a Bonus structure This position is based in Carson City, NV. MA Care Partners specializes in caring for Medicare Advantage members, with a focus on complex, high-acuity geriatric patients. In this role, you will conduct comprehensive health assessments (similar in scope to Medicare Annual Wellness Visits). In addition, over time, you have the opportunity to enroll patients into primary care and build your own panel of Medicare Advantage members, developing longitudinal relationships with the patients you serve. To start, the role is entirely home visit-based, conducting in-home health assessments for Medicare Advantage members across Carson City, Minden, Gardnerville, and surrounding rural Nevada communities. We are planning to open a Carson City clinic within the next few months, at which point the role will transition to a mixture of home visits and clinic-based care. Qualifications to fulfill role as a Nurse Practitioner must include: Must be a licensed Nurse Practitioner (NP) in the state of Nevada - or be eligible for a Nevada nursing license.

Physician Assistants (PAs) are not eligible for this role at this time. Certified by a national Nurse Practitioner certifying body (ANCC or AANP) Strong working knowledge of HCC (Hierarchical Condition Category) coding and risk adjustment methodology, including CMS-HCC and RxHCC models Familiarity with HEDIS measures, Medicare Advantage quality programs, and Stars ratings Experience or strong interest in geriatric medicine and/or Medicare Advantage primary care Comfortable working independently in home visit settings; willingness to transition to a hybrid clinic/home visit model as our Carson City clinic opens Experience conducting comprehensive health assessments, wellness evaluations, or Annual Wellness Visits preferred Commitment to thorough, compliant documentation with emphasis on MEAT criteria and accurate ICD-10 coding to support risk adjustment
