Job Description
Established in 1926, Family Service League is a social services agency transforming lives and communities through mental health, addiction, housing, and essential human services with more than 60 social service programs in over 20 locations.
Family Service League is seeking a part-time Psychiatric Nurse Practitioner for the Assertive Community Treatment (ACT) program. The Psychiatric Nurse Practitioner assesses the mental health status of clients by studying their relevant medical history, performing comprehensive assessments, and identifying risk factors that might affect mental health. This position diagnoses patients mental illness and determines effective plans of care. This position provides ongoing home visits with clients in the community, including checking vitals and administering medications. Travel is required.
Administer and monitor medications, including injectable medications.
Provide medical assessments and vitals for consumers in the community.
Conduct health assessments of adults with potential acute and/or chronic health problems.
Initiate referrals to specialty services and foster continuity of care.
Initiate selected therapeutic and diagnostic procedures, and consultation and laboratory examinations.
Provide patient/family education and counseling in the areas of health promotion, maintenance, and disease prevention.
Participate in team meetings and conferences to ensure an interdisciplinary approach to primary health care delivery.
Serve as liaison between patient and community, and act as a health care advocate.
Identify quality improvement issues and document standards of care and practice.
Maintain a current level of knowledge relative to professional practice.
Act as a resource to staff and implement educational programs.
Participate in the evaluation of the cost effectiveness of services provided.
All other duties as assigned.
Ability to work with a multi-disciplinary team in providing treatment to persistent, severe mentally ill population required.
Valid and clean NYS Drivers License required.

RESPONSIBILITIES
Prescribe and revise medical regimens; interpret findings and plans of care according to collaborative practice agreements and protocols in designated areas.Administer and monitor medications, including injectable medications.

QUALIFICATIONS
Nurse Practitioner degree and license, including state licensure and DEA credentials, are required. A minimum of 3 years of prior prescribing experience is required. Psychiatric prescribing experience is preferred. Proficient computer skills, including Microsoft Office required; experience with an EMR is preferred. Excellent time management, interpersonal, and verbal and written communication skills required.Ability to work with a multi-disciplinary team in providing treatment to persistent, severe mentally ill population required.
Valid and clean NYS Drivers License required.
PHYSICAL REQUIREMENTS
