Job Description

About the Position The Nurse Practitioner (NP) or Physician Assistant (PA) provides comprehensive primary care services to patients of all ages, with a strong focus on preventive care, chronic disease management, health promotion, and patient education. Working collaboratively with physicians and an interdisciplinary healthcare team, the provider delivers high-quality, patient-centered care in accordance with New York State regulations and established clinical protocols. This opportunity is with a well-established Federally Qualified Health Center (FQHC) serving Western New York communities through a network of primary care, behavioral health, dental, and specialty care services. The organization is dedicated to improving access to quality healthcare for underserved populations and plays a critical role in addressing health disparities throughout the region. Responsibilities Provide comprehensive primary care services, including wellness visits, preventive screenings, diagnosis, and treatment of acute and chronic conditions. Conduct detailed physical assessments and psychosocial evaluations through patient history and examination. Order, interpret, and evaluate laboratory and diagnostic tests. Prescribe medications and implement therapeutic and corrective treatment plans within the scope of practice. Deliver patient and family education focused on disease prevention, health maintenance, and self-management strategies. Collaborate with physicians and other healthcare professionals to coordinate patient care. Supervise and provide clinical guidance to Registered Nurses (RNs), Licensed Practical Nurses (LPNs), Medical Assistants, and other clinical staff. Ensure compliance with clinical policies, infection control standards, confidentiality requirements, and regulatory guidelines. Participate in quality improvement, quality assurance, and patient safety initiatives. Maintain accurate and timely medical documentation and reporting. Precept, mentor, and evaluate NP/PA students as assigned. Adhere to all applicable practice agreements, protocols, and organizational policies. Perform additional duties and special projects as assigned.
Qualifications For All Candidates:
Strong clinical assessment, diagnostic, and patient management skills. Proficiency with electronic health records and Microsoft Office applications, including Word, Excel, and Outlook. Preferred Qualifications DEA registration in New York State. Five (5) or more years of outpatient clinical experience. Previous experience in community health, federally qualified health centers, or underserved care settings. Strong proficiency with electronic medical records and Microsoft Office applications.
Compensation & Benefits Compensation:
$120,000 - $130,000 annually , depending on experience.
