Job Description

Join a value-driven primary care team focused on caring for older adults in Queens. This locum-to-perm role (conversion expected within 3-6 months) blends outpatient clinic work with home-based visits, giving you variety and the chance to build meaningful patient relationships. What you'll do Provide full-spectrum primary care for geriatric patients: preventive care, chronic disease management, and acute visits. Diagnose, treat, prescribe, and create individualized care plans. Order and interpret labs and imaging. Document thoroughly in the EHR and ensure timely follow-up. Educate patients and promote wellness and prevention. Coordinate care with physicians, nurses, and support staff; refer to specialists as needed. Maintain compliance with state, federal, and professional regulations. Work schedule & caseload Full time, guaranteed hours Monday-Friday (5 days/week) Hybrid model: outpatient clinic + home visits Home-based care: about 3-5 patients/day Clinic visits: approximately 7-10 patients/day Low daily census to allow focused, patient-centered care Compensation $75-$95 per hour Requirements Active NP or PA license in New York National board certification Minimum 2 years' primary care experience Medicaid and Medicare provider IDs required Spanish-speaking clinicians preferred Expedited credentialing available If you enjoy a mix of clinic and home-based care and want to work in a collaborative, value-based setting serving older adults, this could be a great fit.
