Job Description

Salary Ranges between: $130k-$180k, varies depending on years of experience An established outpatient healthcare organization in New York is seeking an experienced NP/PA to join a collaborative and mission-driven clinical team serving diverse patient populations. Responsibilities :
Conduct comprehensive patient evaluations, including medical histories, physical examinations, and preventive screenings such as women s health exams and pap smears. Develop individualized care plans, order and interpret diagnostic testing, and prescribe medications and treatment plans as clinically appropriate. Partner closely with physicians, medical assistants, nurses, and support staff to ensure high-quality coordinated patient care. Provide patient education in a clear, compassionate, and culturally sensitive manner to improve long-term health outcomes. Maintain timely and accurate documentation within the electronic medical record (EMR) system. Support chronic disease management through ongoing patient follow-up and continuity of care. Participate in quality improvement initiatives, staff meetings, and collaborative clinical discussions. Coordinate referrals to specialists and community resources when needed. Utilize standing orders and collaborate with clinical support staff to optimize workflow efficiency and patient experience. Foster a positive team culture built on communication, accountability, and continuous learning.
Qualifications :
Active New York State NP or PA license and registration required. Minimum of 2 years of experience practicing as a Nurse Practitioner or Physician Assistant in primary care, family medicine, internal medicine, outpatient, or community health settings. Previous Women s Health or OB/GYN experience highly preferred. Strong communication and interpersonal skills with a collaborative approach to care.
