Per Diem/ Part-Time Nurse Practitioner/Physician Assistant

Nurse Practitioner / Physician Assistant (NP/PA) Community Care Physicians - Capital Region Family Medicine Community Care Physicians (CCP), the largest independent multispecialty medical group in the Capital Region, is seeking a Nurse Practitioner or Physician Assistant to join Capital Region Family Medicine , a growing primary care practice in Latham, NY providing comprehensive, patient-centered care to individuals and families across the lifespan. Position Overview As an Advanced Practitioner at Capital Region Family Medicine, you will deliver full-spectrum family medicine in a collaborative, team-based setting alongside an experienced physician and Advanced Practice Providers. The practice emphasizes preventive care, continuity, and accessible services, supported by modern tools such as online scheduling, telehealth, and a secure patient portal. Responsibilities Conduct comprehensive patient evaluations and manage acute and chronic conditions Order and interpret diagnostic studies, prescribe medications, and coordinate referrals as needed Provide preventive care, wellness visits, and patient education across all ages Collaborate closely with physicians, Advanced Practitioners, and support staff to ensure continuity of care Maintain accurate, timely clinical documentation in accordance with organizational standards Practice Highlights Care across the lifespan - serving patients from infancy through geriatrics in a true family medicine model Strong access model , including same-day appointments for new and existing patients and walk-in vaccination availability Convenient weekday schedule (7:00 AM - 4:00 PM) with a physician on call after hours to support continuity of care Robust digital tools , including online scheduling and a secure patient portal for messaging, results, and care coordination Bilingual care environment , with Spanish-speaking provider support to better serve a diverse patient population Located in Latham's Wellness Way health campus , offering convenient access and proximity to additional CCP services Compensation starting at $125,000 and competitive benefits! License and active registration as a physician assistant or registered nurse/nurse practitioner in New York State.

Physician Assistant:

Bachelor's or equivalent degree in physician assistant studies and successful completion of physician assistant training program approved by the NYS Department of Education or accredited by the Accreditation Review Commission on Education for the Physician Assistant, or a program that is determined by the Department to be the substantial equivalent of a licensure qualifying or accredited program. Attainment of passing grade on the Physician Assistant National Certifying Examination administered by the National Commission on Certification of Physician Assistants. Current certification by National Commission on Certification of Physician Assistants. At least one year of experience as a physician assistant in ambulatory health care. Current CPR certification.

Nurse Practitioner:

Successful completion of and graduation from a nurse practitioner education program acceptable to NYSED. Current certification as a nurse practitioner by the American Academy of Nurse Practitioners Certification Board (AANPCB) or the American Nurses Credentialing Center (ANCC). Successful completion of pharmacology coursework acceptable to NYSED. Three years of professional nursing experience in an ambulatory setting preferred. At least one year of experience as a nurse practitioner in ambulatory health care. Current CPR certification.
