hr. shifts Position Summary The Advanced Practice Provider (Nurse Practitioner or Physician Assistant) will support the Heart Valve Center at St. Francis Hospital, providing comprehensive, patient-centered care to individuals with complex valvular heart disease. The APP will work collaboratively with cardiologists, cardiothoracic surgeons, structural heart specialists, nursing, and multidisciplinary teams to ensure seamless evaluation, treatment planning, procedural coordination, and longitudinal follow-up care. This role is integral to delivering high-quality cardiovascular outcomes and enhancing the patient experience throughout the continuum of care. mitral and tricuspid TEER, and other structural heart interventions. Provide inpatient and outpatient management, including post-procedural follow-up. Collaborate with the multidisciplinary Heart Team in treatment planning and case review conferences. Educate patients and families regarding diagnosis, treatment options, and post-discharge care. Coordinate care transitions to ensure continuity across hospital and outpatient settings. Document clinical findings and treatment plans in accordance with regulatory and institutional standards. Qualifications Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in New York State. Board certification required. Minimum 2 years of cardiology or cardiovascular experience preferred. Structural heart or valve program experience strongly preferred. Strong clinical decision-making and collaborative skills.
