What You'll Do:
As a Family Nurse Practitioner , you will be a vital part of our healthcare team, delivering compassionate, patient-centered care.
Your responsibilities will include:
Health Assessments:
Conduct comprehensive physical and psychological assessments through medical history and physical exams.
Treatment & Care Coordination:
Collaborate with physicians and healthcare professionals to determine appropriate treatment plans for your patients.
Patient Education:
Advise and educate patients and their families on their health, treatment options, and progress.
Community Connections:
Coordinate with community resources and make appropriate medical or social service referrals when necessary.
Telehealth & Home Visits:
Provide telehealth services and conduct home visits for patients with medical and dental needs, whether or not they have web enablement.

Competitive Compensation:
Pay range of $115,000 - $125,000 per year.

Master's Degree or Graduate Degree from an approved NP program. Two (2) years of clinical experience as a Nurse Practitioner preferred.
Strong Communication Skills:
Ability to interact effectively with patients, families, and healthcare teams.
Telehealth Experience:
Familiarity with telehealth services is a plus, but not required.
