Role:
UBMD Primary Care is seeking a skilled and patient‑focused Nurse Practitioner or Physician Assistant to provide primary care services at our Tonawanda and Williamsville clinics. This role is ideal for an APP who thrives in a collaborative, fast‑paced outpatient environment and is committed to delivering high‑quality, patient‑centered care. The APP will work under physician supervision and play a key role in health promotion, disease prevention, patient education, and care coordination.
What You'll Do:
You'll use your clinical expertise, diagnostic skills, and patient‑care abilities to support our providers and ensure excellent care delivery.
Responsibilities include:
Clinical Care & Diagnostics Conduct patient exams and obtain medical histories Interpret diagnostic test results and identify deviations from normal Formulate diagnoses and develop treatment plans Prescribe medications or therapies with physician approval Order or perform diagnostic tests (x‑rays, EKGs, labs) Perform therapeutic procedures (injections, immunizations, wound care, suturing) Patient Education & Care Coordination Counsel patients on treatment plans, preventive care, and health maintenance Assist physicians during complex procedures Coordinate follow‑up care, referrals, diagnostic testing, and prescription requests Provide educational materials and community resource information Clinical Operations & Compliance Maintain accurate EMR documentation Follow PCMH standards and support team‑based care Adhere to HIPAA, OSHA, infection control, and quality assurance policies Participate in clinic meetings and ongoing training Maintain excellent attendance and professional conduct Skills That Lead to

Core Skills:
Strong communication and patient‑interaction abilities Ability to multitask and stay organized in a busy outpatient setting Sound clinical judgment and diagnostic reasoning Professionalism, reliability, and teamwork
Technical Skills:
EMR proficiency (Allscripts & EPIC preferred) Knowledge of diagnostic testing and therapeutic procedures Understanding of preventive care, treatment alternatives, and medication interactions
Qualifications:
Graduate of an accredited NP or PA program Current NYS NP or PA license 2 years of healthcare experience, preferably in outpatient or primary care (preferred) EMR experience strongly preferred Strong clinical knowledge and patient‑care skills Work Environment Fast‑paced, well‑lit outpatient clinic Frequent standing, walking, and patient interaction Light lifting up to 15 lbs May travel to other UBMD Primary Care locations as needed Exposure to typical clinical hazards (infectious diseases, sharps, etc.) Additional Information Employment is contingent upon a successful background check and drug screen.
