Job Description

Job Summary Mohawk Valley Health System (MVHS) is seeking a highly skilled and dedicated Advanced Practice Provider to join our cardiac team as a Structural Heart Coordinator. This role is responsible for coordinating and managing the care of patients undergoing advanced structural heart procedures, including pre-procedure evaluation, inpatient and outpatient management, patient education, and post-procedure follow-up. The Structural Heart Coordinator will work collaboratively with the multidisciplinary Heart Team to implement evidence-based care pathways, optimize patient outcomes, and support the continued growth and success of the Structural Heart Program. REQUIRED
Master's degree from an accredited Nurse Practitioner's Program OR certification from an accredited NP program OR completion of an accredited Physician Assistant Program required. Minimum 1 year of experience in a clinical setting. Ability to meet and maintain credentialing requirements and hospital priviledges. Knowledge and competency in a wide range of clinical areas. Familiar with EHR/EMR software. Licensure/Certification Requirements
REQUIRED
Current NYS Nurse Practitioner licenses OR Current NYS license as a Physician Assistant with prescription privileges. Current
BCLS, ACLS
certifications.
PREFERRED
