Job Description

We are seeking a compassionate, versatile, and dedicated Psychiatric-Mental Health Nurse Practitioner (PMHNP) to join our clinical team. This unique role offers the best of both worlds: providing high-quality, continuous psychiatric care to residents within a skilled nursing/long-term care facility , alongside managing a diverse caseload at our outpatient mental health clinic . The ideal candidate will have a strong foundation in adult and geriatric psychiatry, excellent diagnostic skills, and the ability to seamlessly transition between a fast-paced outpatient setting and a collaborative long-term care environment.
Key Responsibilities Dual-Setting Care:
Split clinical time between the outpatient clinic and designated nursing home facilities (schedule to be mutually agreed upon).
Psychiatric Evaluations:
Conduct comprehensive psychiatric assessments, diagnostic screenings, and mental status examinations for both outpatient clients and long-term care residents.
Medication Management:
Prescribe, adjust, and monitor psychotropic medications, with a strict emphasis on safely managing polypharmacy and age-appropriate dosing guidelines in the geriatric population (e.g., Beers Criteria).
Behavioral Management Plans:
Collaborate closely with nursing home staff to develop non-pharmacological behavioral interventions for residents experiencing dementia, delirium, or severe persistent mental illness (SPMI).
Therapeutic Collaboration:
Partner with outpatient therapists, primary care physicians, nursing home medical directors, and families to ensure holistic, coordinated care.
Documentation:
Maintain precise, timely electronic health records (EHR) adhering to state, federal, and Medicare reimbursement guidelines for both settings.
Qualifications & Requirements Education:
Master's or Doctorate degree from an accredited Nurse Practitioner program with a specialization in Psychiatric-Mental Health (PMHNP).
Licensure & Certification:
Active, unrestricted Advanced Practice Registered Nurse (APRN) license in the state of practice, and Board Certification (PMHNP-BC) via the ANCC.
Credentials:
Valid DEA registration and state controlled substance license.
Experience:
Minimum of 1-2 years of psychiatric experience is preferred. Experience in geriatric psychiatry, long-term care, or consultation-liaison roles is highly valued.
Skills:
Strong communication skills, autonomy in clinical decision-making, comfort working with complex medical comorbidities, and adaptability.
Benefits & Compensation Salary:
[$113,000 to $137,000 ] per year (commensurate with experience).
Malpractice Cover:
Fully covered malpractice insurance (including tail coverage).
Pay:
$114,118.41 - $137,432.92 per year
