This is a fantastic opportunity to build your career while serving a diverse patient population in both primary care and mental health!
What You'll Be Doing:
Conduct comprehensive patient evaluations, including physical exams and health assessments Collaborate with physicians and specialists to ensure coordinated, well-rounded care Create and manage individualized treatment plans tailored to patient needs Prescribe medications and monitor treatment effectiveness Support patients with both primary care and behavioral health concerns Coordinate referrals to specialty providers and external services Contribute to the development and improvement of clinical care processes
Requirements:
Active New York State license as a Family Nurse Practitioner (FNP) or Physician Assistant (PA) New grads are welcome to apply! At least 1 year of primary care/clinical experience preferred Bilingual English/Spanish is a plus
