Job Description

Job Overview We are seeking a highly skilled and compassionate Advanced Practice Provider (Nurse Practitioner or Physician Assistant) for our Primary Care Team. This role involves delivering high-quality patient assessments, managing acute and chronic conditions, and supporting our mission to provide effective, affordable mental health and medical services within a community setting. This position is contract, seeking 8 to 16 hours of weekly coverage (Monday - Friday, no holidays and no weekends). Key Responsibilities Provide comprehensive primary care services including assessment, diagnosis, and treatment of acute and chronic conditions Conduct physical exams and obtain medical histories Order and interpret laboratory and diagnostic tests Prescribe medications and treatments in accordance with state and federal regulations Provide preventive care including wellness exams, screenings, and immunizations Manage chronic diseases such as diabetes, hypertension, and asthma Educate patients on disease prevention, lifestyle modifications, and medication adherence Collaborate with physicians, behavioral health providers, dentists, and care coordinators in a team-based care model Document care accurately in the electronic health record (EHR) Participate in quality improvement initiatives and population health efforts Qualifications Graduate of an accredited Nurse Practitioner (NP) or Physician Assistant (PA) program Current state licensure and national certification Current DEA license or eligibility Current BLS certification Strong commitment to serving medically underserved populations Experience in family medicine, community health, or primary care (preferred)
