Job Description

Job Summary Hospice of Guernsey, Inc. is seeking a dynamic and compassionate Nurse Practitioner to join our healthcare team, dedicated to delivering exceptional patient care across diverse clinical settings. In this role, you will serve as a vital member of the medical team, providing comprehensive assessments, diagnosing conditions, and managing treatment plans with a focus on patient-centered care. Your expertise will help improve health outcomes, support chronic disease management, and promote wellness through evidence-based practices. This position offers an exciting opportunity to work in a fast-paced environment where your skills in acute and primary care are highly valued. Responsibilities Conduct thorough physical examinations and patient assessments across various age groups, including pediatrics, geriatrics, and adults. Develop and implement individualized treatment plans, including medication management Manage chronic conditions Collaborate with multidisciplinary teams to coordinate discharge planning, case management, and utilization review for inpatient and outpatient populations. Maintain accurate medical documentation within EMR (Electronic Medical Records) while ensuring compliance with HIPAA regulations. Provide health coaching and education tailored to individual needs in areas such as geriatrics, hospice & palliative medicine, behavioral health, or working with individuals with disabilities. Skills Familiarity with CPT coding for billing purposes and ICD coding standards (ICD-9 & ICD-10).
