Deliver high-quality care and make a meaningful impact on patients' lives as a Nurse Practitioner performing home health risk assessments within Summit County. This per diem opportunity offers flexibility and autonomy for experienced Nurse Practitioners who value work-life balance and professional fulfillment.

Schedule:

Flexible shifts available Monday through Sunday Self-scheduling—build your own workload Minimum weekly or bi-weekly hourly commitments

Key Responsibilities:

Conduct annual home health risk assessments for Medicare Advantage plan members in a home setting Evaluate comprehensive medical and medication histories Educate members on their health conditions and medication management Recognize care gaps and provide informed recommendations to improve quality outcomes Complete thorough documentation—no treatment plan development or prescribing required

Desired Skills & Experience:

Unrestricted Nurse Practitioner License in Ohio MSN from an accredited school of nursing Board Certification as NP through AANP or ANCC Current BLS/CPR certification At least 6 months of recent NP experience preferred Excellent interpersonal and time management skills Strong commitment to patient-centered care In this role, you'll receive $80-$90 per evaluation, including a mileage stipend and compensation for administrative time.
