Job Description

Overview Medicine Inpatient Group is physician owend group with both hospitalists and SNF physicians and midlevel providers is seeking a dedicated and skilled Nurse Practitioner to join our healthcare team to work in SNF/ nursing home setting. The ideal candidate will possess a strong clinical background in nursing home and skilled nursing facility and demonstrate a commitment to providing high-quality patient care. As a Nurse Practitioner, you will play a crucial role in assessing, diagnosing, and treating patients ensuring adherence to medical standards and best practices. While enjoying independence, physician back up is always available. Duties Conduct comprehensive patient assessments, including physical examinations and medical histories. Develop and implement individualized care plans based on patient needs and clinical guidelines. Perform medical documentation accurately to ensure continuity of care and compliance with healthcare regulations. Collaborate with interdisciplinary teams to provide holistic care. Monitor patient progress and adjust treatment plans as necessary. Provide education to patients and families regarding health management, preventive care, and treatment options. Qualifications Master's degree in Nursing or equivalent qualification as a Nurse Practitioner. Valid Nurse Practitioner license in the Ky and Ohio. OH is a must, May apply to Ky after hiring. Proficiency in patient assessment techniques and medical documentation practices. Experience working in nursing homes is preferred. Excellent communication skills and the ability to work collaboratively in a team-oriented environment.
