Job Summary:
We are seeking a friendly and compassionate Pulmonary Nurse Practitioner/ Physician Assistant to join our growing team. In this role, you will provide comprehensive pulmonary care to patients in post-acute care settings while collaborating closely with physicians, nursing staff, and interdisciplinary care teams. This is an exciting opportunity to make a meaningful impact while working in a supportive and collaborative clinical environment focused on improving respiratory outcomes and enhancing patients quality of life.
Responsibilities:
Conduct pulmonary assessments for patients in post-acute care settings Evaluate, diagnose, and manage pulmonary conditions including COPD, Asthma, and more. Evaluate, treat, and manage patients experiencing acute or worsening medical conditions requiring timely intervention Evaluate, diagnose, and manage pulmonary conditions including COPD, asthma, pneumonia, and respiratory failure Develop and implement individualized treatment plans Collaborate with interdisciplinary teams Educate patients, families, and facility staff on respiratory care and treatment plans Monitor patient progress and adjust care plans as needed Maintain accurate and timely EMR documentation Support initiatives focused on reducing hospitalizations and Return-to-Hospital (RTH) events Requirements Preferred a minimum of 6 months of experience in post-acute care setting Preferred experience in
Pulmonary:
training provided Active and unrestricted state Nurse Practitioner license or Active PA license and NCCPA certification required Master s Degree in Nursing from an accredited Nurse Practitioner program or Master s Degree from an accredited Physician Assistant program required.
CPR BCLS/BLS
certified Strong communication, organizational, and teamwork skills Ability to work independently in the clinical environment
