Position Description:
We are looking for a dedicated Nurse Practitioner in Warren, OH. This is a full-time, in-person position, Monday - Friday 9am - 5pm. Qualifications of the
Nurse Practitioner:
Active Nurse Practitioner license in the state of Ohio, in good standing. Active DEA registration. 1+ years of experience as a Nurse Practitioner; SNF, geriatric, or post-acute experience preferred. Strong clinical documentation skills and familiarity with EMR systems. Excellent communication skills with patients, families, and interdisciplinary teams. Responsibilities of the
Nurse Practitioner:
Examine residents upon admission and review medical records to establish baseline assessments. Conduct routine follow-up visits and manage chronic conditions including diabetes, hypertension, CHF, COPD, and dementia. Prescribe and adjust medications including controlled substances per state regulations. Lead transitional care management and discharge planning with social work, nursing, and rehabilitation teams. Engage with residents' families on care plans, goals of care, and advance directives.
