Job Description

Summary Might be eligible for the Education Debt Reduction Program (EDRP)
  • a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval
  • award amount (up to $200
  • 000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application.

  • but are not limited to: Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice Develops and implements an individualized plan to achieve optimal population health and disease management Demonstrates leadership and works collaboratively with Primary Care Team and other interdisciplinary team members Orders and interprets labs•tests•and diagnostics studies Prescribes pharmacologic and non-pharmacologic interventions Initiates referrals and consults to specialty services Provides education to patients and their families Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes Exemplifies the ICARE core values of-integrity•Commitment•Advocacy•Respect•and Excellence This position potentially requires flexibility in schedule Driving and travel for Primary Care patient care coverage purposes will be required for this position to clinics throughout the health care system Driving and travel may be up to 75% of the time All other duties as assigned VA offers a comprehensive total rewards package:
