Job Description

Job Summary We are seeking a dedicated and energetic Part-Time Nurse Practitioner specializing in Family Practice to join our dynamic healthcare team. In this role, you will provide comprehensive primary care services to patients across all ages, focusing on health promotion, disease prevention, and management of acute and chronic conditions. Your expertise will help improve patient outcomes through compassionate care, clinical assessment, and collaborative treatment planning. This position offers flexibility for professionals passionate about making a positive impact in diverse community settings while maintaining a balanced work-life schedule. Duties Conduct thorough physical examinations and patient assessments to determine health status and develop individualized care plans. Manage acute and chronic illnesses utilizing evidence-based practices, including medication administration and treatment adjustments. Utilize EMR (Electronic Medical Records) systems like Epic, eClinicalWorks, or Cerner to document patient encounters accurately and efficiently. Collaborate with multidisciplinary teams on discharge planning, case management, and utilization review to ensure comprehensive patient care. Provide health coaching, education on disease prevention, family planning, and lifestyle modifications tailored to patient needs. Participate in telehealth consultations and triage services to extend access to quality care beyond traditional clinic hours. Qualifications Current Nurse Practitioner license with certification in Family Practice from an accredited program. Proven experience working in outpatient clinics or medical offices with familiarity in Epic or other EMR/EHR systems such as Athenahealth or eClinicalWorks. Strong knowledge of medical terminology, physiology, anatomy, CPT coding, ICD-10/ICD-9 coding systems for accurate documentation and billing. Experience managing diverse patient populations including pediatrics, geriatrics, assisted living residents, or individuals with developmental disabilities. Critical care experience preferred; familiarity with urgent care, emergency medicine, or hospital medicine is a plus. Knowledge of HIPAA regulations ensuring patient confidentiality and data security at all times. Excellent communication skills for effective patient education and team collaboration in fast-paced environments such as Level I trauma centers or teaching hospitals.
