Job Description

Primary Care Nurse Practitioner (APRN/NP)
  • Behavioral Health Setting | $150K+ Potential | No Mental Health Caseload Community Healthcare Partners is actively hiring a Full-Time Nurse Practitioner (APRN/NP) to deliver high-quality primary care services within behavioral health facilities in Oregon City and SE Portland .
This is a unique opportunity to focus strictly on medical care — no mental health treatment responsibilities —while working alongside a fully staffed, on-site behavioral health team. We are seeking a confident, experienced provider who thrives in a hands-on, facility-based setting and can independently manage a wide range of patient conditions.
Position Highlights Full-Time Flexible Scheduling:
5x8s or 4x10s (day shifts only)
Full-Time Compensation:
Base salary starting at $90,000+ Uncapped bonus structure Total earning potential: $150,000+ annually Facility-based care only (no home visits, no telehealth) Supportive on-site staff present during all patient visits 2-year commitment required Start date following credentialing What You'll Do Perform physical exams and review patient histories Diagnose and treat acute, chronic, and infectious conditions Order and interpret labs and diagnostic tests Prescribe and manage medications Provide patient and family education on prevention and wellness Maintain accurate EMR documentation and scheduling (EClinical Works) Collaborate with interdisciplinary team Deliver proactive, patient-centered primary care Work Environment Advantage No mental health caseload
