Build an empaneled patient panel and provide comprehensive, relationship-based care that supports long-term health outcomes.Team based care environment:
Work alongside NPs, MAs, nurses, care coordinators, behavioral health providers, lab staff, and interpreters.What You'll Do:
Serve as an empaneled primary care provider for new and established Wallace patients across the lifespan. Provide preventive care, health screenings, chronic disease management, and patient education to support whole-person wellness. Perform clinical evaluations, physical exams, and histories. Order and interpret diagnostic tests and labs. Collaborate with the Care Team to develop longitudinal care plans and coordinate referrals, follow-up, and preventive services across the care continuum. Partner closely with RNs, MAs, behavioral health, care coordination, and other interdisciplinary team members to deliver integrated care. Maintain high quality documentation, follow up planning, and patient education. Maintain a patient-centered approach that advances access, continuity, and quality outcomes for the communities we serve.What We're Looking For:

Current, unrestricted NP license in Oregon Current, unrestricted DEA license Current BLS certificate Experience managing chronic conditions, preventive care, and complex primary care needs across diverse patient populations Commitment to team based, patient centered carePreferred:
Experience in community health, family practice, FQHC, or other outpatient primary care settings. Experience supporting Medicaid or higher acuity primary care populations Bilingual in Spanish (preferred, but not required) Collaborative, team oriented, and grounded in patient centered values Able to balance efficiency with thoughtful follow-up and sustained patient relationship-building.
