Primary Care Functional Medicine Nurse Practitioner or Physician Assistant At OnePeak Medical, we are redefining primary care through a personalized, integrative model that blends traditional and functional medicine. RESPONSIBILITIES AND DUTIES

Provide direct patient care through office visits, telehealth, and occasional same-day appointments Performprehensive assessments, order and interpret labs/imaging, and diagnose acute and chronic conditions Develop personalized treatment plans that may include lifestyle interventions, supplements, hormone therapies, and prescriptions Educate and empower patients on preventive health and functional medicine principles Collaborate with internal health coaches, nutritionists, mental health providers, and clinical support staff Document visits in the EHR (NextGen) in a timely andpliant manner Participate in team meetings, quality improvement initiatives, and training sessions Support clinic operations by contributing to a positive, team-oriented environment

REQUIRED QUALIFICATIONS

Active Oregon license as a Nurse Practitioner (FNP, AGNP, or AGNP-PC), PA-C, MD, or DO (or eligibility to obtain) Board certification in Family Medicine, Internal Medicine, or relevant specialty Minimum 2 years of clinical experience preferred (functional/integrative medicine experience a plus) Strong interest in preventive, functional, and lifestyle medicine Excellentmunication and patient relationship skills Comfortable working in a fast-paced, collaborative environment Proficient with electronic health records (NextGen experience a plus)
