Job Description We are seeking a skilled and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to support facilities in Pittsburgh , PA. This role focuses on providing high-quality care to patients in skilled nursing and assisted living facilities, primarily within post-acute and long-term care settings. Position Details Part-time, per diem role (approximately 2-4 shifts per month) Hybrid model (in-person care + remote patient monitoring) Flexible scheduling based on clinical needs Competitive per-patient compensation based on experience and volume Opportunity to make a meaningful impact on patient outcomes Key Responsibilities Conduct comprehensive patient assessments and physical examinations Develop and manage treatment plans in collaboration with supervising physicians Review and interpret laboratory results and diagnostic data Provide patient and family education on health management and chronic conditions Document patient care accurately in electronic medical records (EMR) Coordinate care with interdisciplinary teams and facility staff Support ongoing patient monitoring and follow-up care Qualifications Active and unrestricted NP/PA license in PA.
