• Full-time Position Summary Berks Community Health Center (BCHC) seeks a CRNP to provide comprehensive, patient-centered care to individuals and families of all ages.

Collaborating with physicians, the CRNP leads a care team, manages acute and chronic conditions, and ensures preventive care. Responsibilities include coordinating patient care, directing ambulatory services, supporting quality improvement initiatives, participating in peer review, and contributing to BCHC's designation as a Federally Qualified Health Center (FQHC) and Patient-Centered Medical Home (PCMH). The CRNP champions BCHC's mission to deliver exceptional primary and preventive healthcare to all Berks County residents, regardless of ability to pay. Principal Duties and Responsibilities Provide high-quality, evidence-based, patient-centered primary care to all patients. Lead and mentor care teams, including clinical and non-clinical staff. Diagnose, treat, and manage acute and chronic conditions; prescribe medications and develop treatment plans. Ensure accurate, confidential documentation and adherence to regulatory, accreditation, and clinical standards. Collaborate with physicians, specialists, and care team members to coordinate patient care and optimize outcomes. Participate in quality improvement initiatives, peer review, and patient safety projects. Educate patients and families on prevention, self-management, and health maintenance. Support BCHC's mission, FQHC and PCMH designations, and initiatives to improve workflow, patient care, and team effectiveness.
