In this vital role, you will provide comprehensive primary care services, including health assessments, diagnosis, treatment, and health education to diverse patient populations across all ages. Your expertise will support patients in achieving optimal health outcomes through proactive management, patient-centered care, and collaboration with multidisciplinary teams. This position offers an exciting opportunity to make a meaningful impact in community health settings while utilizing your broad clinical skills and specialized knowledge. Duties Conduct thorough physical examinations and health assessments for patients of all ages, including infants, children, adults, and seniors. Develop and implement individualized treatment plans based on diagnostic evaluations, patient history, and current best practices. Administer medications, including injections and IV infusions, and perform procedures such as suturing, dermal fillers, and esthetic laser treatments. Manage acute and chronic conditions across various specialties such as gastroenterology, geriatrics, behavioral health, infectious disease care, and pain management. Utilize electronic health record (EHR) systems like Epic or eClinicalWorks to document patient encounters accurately while ensuring compliance with HIPAA regulations. Coordinate care with specialists for complex cases involving ICU or Level I trauma center experience, dialysis patients, or those requiring post-acute or hospice & palliative medicine services. Engage in telehealth consultations to expand access to care while providing patient education on wellness strategies including nutrition, memory care, or family planning. Skills Proven proficiency with EMR/EHR systems such as Epic, Cerner, or Athenahealth for efficient documentation and patient management. Strong clinical skills in performing diagnostic evaluations using laboratory testing, sonography, or medical imaging techniques. Expertise in procedures like phlebotomy, catheterization, tube feeding management, sterile processing techniques, and airway management. Knowledge of CPT coding and ICD coding (ICD-9/10) for accurate billing and utilization review processes. Experience working with diverse populations including pediatric (pediatrics), geriatric (geriatrics), assisted living residents, nursing home patients, or individuals with developmental disabilities. Ability to handle triage situations effectively in urgent care or emergency room settings while managing acute pain or infectious disease cases. Familiarity with occupational health protocols such as Workers' Compensation law or post-acute discharge planning to ensure comprehensive patient care delivery.
