Position Overview The Family Nurse Practitioner will provide comprehensive, patient-centered primary care across the lifespan, including preventive care, chronic disease management, acute care, health education, and care coordination. Key Responsibilities
• Deliver high-quality primary care services for children and adults.
• Diagnose, treat, and manage acute and chronic conditions.
• Provide preventive screenings, immunizations, and health education.
• Address social determinants of health in partnership with community resources.
• Engage patients in shared decision-making.
• Document care in EHR and contribute to quality improvement initiatives.
• Support population health and care gap closure efforts. Qualifications
• MSN, DNP, or equivalent advanced nursing degree.
• Active, unrestricted DC Family Nurse Practitioner license.
• ANCC or AANP certification.
• 2-5 years of NP clinical experience preferred, new graduates welcome to apply
• Experience in community health or Medicaid populations strongly preferred.
• Proficiency with EHR systems.

Location & Schedule Part-time, based in C3Cares place-based clinics within DC public housing communities. Telehealth days may be considered as part of the provider schedule following onboarding and based on clinical, operational, and performance needs.
