We are working on an exciting new opportunity with a growing healthcare company that is seeking Nurse Practitioners or Physician Assistants in Madison, GA to work visiting patients at home and virtually. - Hybrid, full time schedule: Monday through Friday 8:30
- 5:00 pm
- Home care or hospice experience preferred
- Chronic disease management experience preferred
- Must be willing to travel up to one hour from home
- Must be able to visit patients in their homes up to 4 days/week, 1 day remote Pay flexible
