This mission-driven organization offers providers the opportunity to make a real difference in their community every day. Key Responsibilities for the
Family Medicine Nurse Practitioner:
Provide compassionate, individualized primary care to patients in an outpatient setting.

Deliver care both in person and virtually, building and managing a panel of patients.

Collaborate with colleagues, community partners, and emergency medical/psychiatric service providers to ensure comprehensive care.

Participate in after-hours on-call rotation 3 - 4 times per year.

Efficiently document within an electronic health record and conduct telemedicine visits. Key Requirements for the
Family Medicine Nurse Practitioner:
Board-certified APRN with a current valid license to practice in the State of Texas.

Passion for serving underserved populations who face significant barriers to care.

Mission-focused, proactive, and able to thrive in a fast-paced setting.

Comfortable working independently while also collaborating with colleagues.

Strong communication and technology skills to support patient care and virtual collaboration. Compensation and Benefits for the
Family Medicine Nurse Practitioner:
