Locums-to-Perm Nurse Practitioner or Physician Assistant Northeast Healthcare Recruitment•4.0 Ridgewood, NY Job Details Full-time | Locum tenens $75•$95 an hour 22 hours ago Qualifications Advanced practice provider General practice Full Job Description We are seeking an experienced Nurse Practitioner or Physician Assistant for a Locums assignment to convert to Perm (within 3-6 months) at a primary care practice serving geriatric patients in a value-based care model. This is a hybrid role with blend of coverage in outpatient clinics as well as home-based patient care.
Compensation:
$75•$95 per hour

Schedule:
Full Time Hours Guaranteed Monday•Friday•5
Days Per Week Patient Load:
LOW DAILY CENSUS
Home Care:
3-5 patients per day
Clinic:
Small census of 7-10 patients per day
Setting:
Hybrid (Outpatient clinic + home-based care), value-based primary care for elderly patients
Responsibilities:
Deliver comprehensive primary care services, including preventive care, chronic disease management, and acute care to Geriatric patients Diagnose, treat, prescribe medications, and develop individualized treatment plans Order and interpret diagnostic tests (labs, imaging, etc.) Document accurately in the EHR system Provide patient education and promote wellness initiatives Collaborate with physicians, nurses, and support staff to ensure coordinated, high-quality care Refer patients to specialists and manage follow-ups Ensure compliance with state, federal, and professional regulations
Requirements:
Active NP or PA license in NY Minimum 2 years of primary care experience National Board Certification Spanish Speaking Preferred Expedited Credentialing Medicaid and Medicare Provider Id required
