Role:

We are seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.

Responsibilities:

Build strong relationships with the facility and maintain a patient-centered approach Conduct on-site visits and provide remote care to residents Document patient visits and maintain accurate records Collaborate with other healthcare professionals to provide comprehensive care Participate in Continuous Quality Improvement (CQI) initiatives to enhance patient outcomes

Requirements:

Active NP or PA license in the state of practice 1+ year of experience in: Geriatric care Documentation review Excellent communication, collaboration, and teamwork skills Masters degree in
