Full Job Description Job Summary:
We are seeking a compassionate and skilled Nurse Practitioner to provide comprehensive care to our PACE participants across various settings in Woonsocket area, including clinics, nursing homes, and homes. You will perform initial and ongoing assessments, develop individualized care plans, and monitor health changes to ensure appropriate management. Your role includes conducting physical, functional, and mental evaluations, interpreting diagnostic tests, and providing preventive, episodic, and chronic care in collaboration with physicians. In this position, you'll manage common health issues, adjust medications, and offer health education and counseling to participants and their families. Your clinical judgment will help differentiate normal aging from pathological conditions, guiding treatment decisions and referrals as needed. You'll also coordinate care, monitor participant progress, and assist with discharge planning, ensuring continuity and quality of care across all care settings. Proficiency in documentation, computer skills, and teamwork are essential. You will participate in care planning, participant conferences, and organizational committees, maintaining confidentiality and thorough records in the electronic health record.
