IL - LaSalle County Cluster Job Description We are seeking a dedicated Nurse Practitioner (NP) to provide high-quality care in post-acute settings. In this role, you will support patients during their recovery following hospital discharge, helping to reduce readmissions and improve overall outcomes. This position offers a structured weekday schedule, autonomy in practice, and a collaborative team environment.Key Responsibilities:
Conduct rounds in assigned facilities on a consistent weekday daytime schedule. Perform focused patient assessments to monitor recovery, manage pain, wounds, and functional decline. Collaborate with physicians, therapists, and facility staff to develop individualized care plans. Complete documentation using an electronic health record (EHR) system (with remote flexibility). Identify and address readmission risks and gaps in post-acute care. Ensure compliance with applicable clinical and regulatory standards.What We Offer:

Master's degree in Nursing with certification as a Nurse Practitioner (NP) from an accredited program. Active, unrestricted NP license with prescriptive authority in the state of practice. 2-3 years of experience in PM&R, geriatrics, post-acute care, or a related field preferred. Experience working in nursing facilities is a plus. Strong clinical skills in managing recovery, rehabilitation, pain, and chronic conditions. Excellent communication and time-management skills.
