Psychiatric Mental Health Nurse Practitioner, PMHNP opening!
Coverage Area:
Williamstown, KY Schedule:
2 nonconsecutive weekdays (M-F), daytime hours — you choose the days!
Compensation:
W2, pay-per-encounter (fee-for-service role) We are looking for experienced Psychiatric Mental Health Nurse Practitioners, PMHNPs for "SNF-ists," in-house nurse practitioners that will provide care for the geriatric population in a Skilled Nursing and/or Assisted Living setting. Telos Health Systems staffs Psychiatric Mental Health Nurse Practitioners, PMHNPs and General Medicine Nurse Practitioners, to improve residents' quality of life and overall well-being while simultaneously reducing return to hospital stays. What You Will Do As the SNF-ist, you will act as a Skilled Nursing Facility's leading provider of internal medicine services. You will collaborate with the facility's interdisciplinary team and physicians, serving as a day-to-day clinical leader while helping oversee the care of all the facility's residents. You will work with the entire facility's patient population - short and long-term residents - overseeing their day-to-day care with the assistance of the facility's nursing staff and guidance from the facility's medical directors and attending physicians. Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease. Prescribe all necessary medications and treatments. Perform comprehensive physical examinations of patients. Develop and implement patient management policies and procedures. Facilitate referrals to other healthcare professionals and medical facilities. Communicate with attending physicians and/or specialists regarding patient care. Be comfortable and proficient with documentation and CPT coding. Monthly stability visits for long term residents Evaluations for patients exhibiting changes in condition. Advanced Care Planning What You Will Need Certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP) Must have an active license and DEA # Previous experience in nursing or other medical fields Minimum one-year experience in a Skilled Nursing setting (preferred) Ability to build rapport with patients and their loved ones. Ability to thrive in a fast-paced environment.
