Make Every Patient Matter — NP/PA Geriatrics Opportunity in Maine About the Opportunity Are you a Nurse Practitioner (NP) or Physician Assistant (PA) looking to make a meaningful impact in the lives of older adults while maintaining a strong work-life balance? This opportunity offers the chance to join a forward-thinking healthcare team delivering patient-centered geriatric care across assisted living, memory care, and long-term care settings throughout Maine. This care model combines in-person clinical care with telemedicine and remote patient monitoring, allowing providers to deliver proactive, evidence-based treatment that improves outcomes and quality of life. What You'll Do Provide comprehensive medical care to older adults in residential care settings Manage chronic conditions, acute issues, and routine regulatory visits Develop and oversee individualized care plans focused on safety, function, and quality of life Collaborate with nursing staff, facility leadership, and interdisciplinary teams Participate in telemedicine visits to support continuity of care Use remote patient monitoring tools to deliver proactive care Complete timely and accurate documentation in the EHR What You Bring Active NP or PA license in the state of Maine (or ability to obtain) Experience in geriatrics, long-term care, or post-acute care preferred, or strong interest in the field Strong clinical assessment and decision-making skills Ability to work independently within a collaborative team model Comfort using EHR systems and telehealth technology Clear and compassionate communication skills Why This Role Stands Out Competitive compensation: $90/hr No weekends or overnight call Predictable schedule supporting work-life balance Opportunities for mentorship, leadership, and professional growth Collaborative and supportive clinical environment Innovative approach to geriatric care delivery Make a Difference This role offers the opportunity to be part of a modern, patient-centered approach to geriatric care, focused on improving outcomes and enhancing the lives of older adults.
