Wound Care Nurse Practitioner(NP)/Physician Assistant(PA)

Position Overview We are seeking an APP who is looking for something different than the traditional hospital or corporate healthcare environment. This role offers a high degree of clinical autonomy, flexibility, and independence . As a Wound Care provider with HWMS, you will manage your own patient panel, structure your daily schedule, and provide care in a way that prioritizes outcomes. This is an ideal role for a provider who is confident in their clinical decision-making, values patient relationships, and prefers a more flexible and self-directed work environment. Care Coordination Collaborate with home health agencies, skilled nursing facilities, and referring providers Communicate treatment plans and updates to the broader care team Support transitions of care following hospital or SNF discharge Educate patients and caregivers on wound management and prevention Documentation & Compliance Maintain accurate, timely, and compliant documentation Ensure documentation meets Medicare and payer requirements Follow HWMS clinical protocols and workflows Participate in periodic chart reviews and quality initiatives QualificationsRequired Active, unrestricted North Carolina license Board Certification (FNP, AGNP, PA-C, or similar) DEA license (or ability to obtain) Valid driver's license and reliable transportation Preferred Wound care experience or certification (CWOCN, CWON, WCC, or similar) Experience in home health, SNF, or post-acute care settings Familiarity with Medicare documentation and billing practices Compensation & Benefits Base salary: $110,000
