If you are a nurse practitioner that wants to practice medicine in a model of care that inspired your initial interest in helping others, PACE-RI wants to have a conversation with you. PACE-RI is an all-inclusive health plan for eligible older adults who wish to remain living in their home and community. The organization provides medical, social, nutritional, rehabilitation and support services for adults 55 and older who otherwise may need nursing home level of care. Because PACE-RI is both the insurer and provider of care for participants, our providers enjoy great flexibility with care planning and delivery that is not often found within other care models. Woonsocket is PACE-RI's fastest growing site. The space is light and bright, drawing its energy from the diverse culture of the community. With its inviting communal spaces and outdoor mural, this site provides a warm and welcoming community for staff and participants. This is a full-time, first shift, Monday-Friday position with on-call rotational responsibilities about once per week. PACE-RI providers meet our participants where the need is so clinical providers must be willing to do home and/or facility visits as needed. Majority of work will be clinic based. Position Requirements:
Master's Degree, Required 2 Years of direct clinical work, Preferred 1 Year of experience in a Medical or Geriatric Care setting, Preferred 1 year of experience working with a frail or elderly population, Preferred RI Advanced Practice Registered Nurse (APRN) and Controlled Substance Registration (CSR-APRN) Graduate from an APRN graduate or post-graduate program recognized by the US Secretary of Education and/or the Council for Higher Education Accreditation Certified by the Board in the APRN role and population focus appropriate to education preparation. CPR Certification Driver's License & access to reliable transportation
