Role:
Join our dynamic team as a Nurse Practitioner, where you'll play a vital role in improving the health and well-being of our patients. In this position, you'll have the opportunity to make a real difference in the lives of those managing kidney and hypertension related conditions.
Responsibilities:
Conduct comprehensive patient assessments and develop individualized care plans. Perform physical examinations and interpret diagnostic tests. Manage and prescribe medications for patients with chronic kidney disease and hypertension. Educate patients and families on disease management and treatment options. Collaborate with a multidisciplinary team to ensure optimal patient outcomes. Document patient care activities and maintain accurate medical records. Participate in quality improvement initiatives and patient safety programs. Stay current with best practices and advancements in nephrology and hypertension care.
Requirements:
A valid Nurse Practitioner license in South Carolina. Excellent communication and interpersonal skills to engage with patients effectively. Ability to work collaboratively in a fast-paced healthcare environment. Detail oriented with strong organizational and time management skills. Commitment to continuous learning and professional development.
