  • Full-time Job Summary We are seeking a dynamic and compassionate Family Nurse Practitioner (FNP) specializing in Primary Care to join our dedicated healthcare team.
In this vital role, you will provide comprehensive, patient-centered care across diverse populations, including pediatrics, geriatrics, and adults. Your expertise will help promote health, prevent illness, and manage acute and chronic conditions in a variety of clinical settings. This position offers an exciting opportunity to make a meaningful impact on patients' lives through proactive health management and personalized care. Duties Conduct thorough physical examinations and patient assessments to evaluate health status and identify medical issues. Develop and implement individualized treatment plans, including medication administration, injections, suturing, and wound care. Utilize EMR (Electronic Medical Record) systems like eClinicalWorks to document patient encounters accurately and efficiently. Provide health coaching and education on disease prevention, medication adherence, nutrition, and lifestyle modifications. Coordinate care with specialists in areas such as gastroenterology, infectious disease, dermatology, and hospice & palliative medicine. Participate in triage activities via telehealth or in-person visits to prioritize urgent cases and ensure timely patient care. Maintain compliance with HIPAA regulations and ensure the confidentiality of all patient records. Qualifications Valid Family Nurse Practitioner (FNP) certification with current state licensure. Proven experience in primary care settings with proficiency in medical office procedures and EMR/EHR systems. Demonstrated knowledge of clinical research methodologies and familiarity with CPT coding, ICD-10 coding standards. Experience working with diverse populations including pediatrics, geriatrics, assisted living residents, nursing home patients, or individuals with developmental disabilities. Skills in performing procedures such as suturing, phlebotomy, venipuncture, catheter care, sterile processing techniques, and basic life support certification. Strong understanding of physiology, anatomy knowledge, diagnostic evaluation techniques including sonography or laboratory experience preferred. Ability to handle complex cases involving acute pain management, infectious disease care, trauma (Level I/II trauma center experience), or post-acute care scenarios like hospice or memory care. Excellent communication skills for patient education and collaboration with multidisciplinary teams across inpatient or outpatient settings.
