Job Description

WHO WE ARE
State of Franklin Healthcare Associates is a physician-led and team member-owned multi-specialty care group headquartered in Johnson City, TN with locations in the upper East Tennessee & Southwest Virginia region. PRIMARY RESPONSIBILITIES
Performs assessment, by obtaining patient medical and social histories, and by performing diagnostic testing and taking vital signs. Evaluates signs and symptoms of patient and orders diagnostic tests. Interprets and evaluates diagnostic tests and laboratory results. Monitors and reviews patient care and treatment plans, which may include nutrition and exercise plans. Provides education and counseling of patients and family in preventive care, medical conditions, and use of prescribed treatments and drugs. Attends staff meetings and serves on committees as needed. Participates in quality assurance activities. Refers patients to physicians or other health care providers and facilities as needed and monitors outcome of the referral. Participates in professional development activities and maintains professional affiliations. Maintains patient confidentiality. Oversees clinical procedures performed by nursing staff in the home setting. Works collaboratively with Physicians in the home setting to assess high risk patients, prescribe treatment plans, and refer to higher level of care as needed. Manage and treat chronically and acutely ill patients per protocol with IV fluids, IV and IM Medications, respiratory treatments, and other approved treatment methodologies.
REQUIREMENTS
Nurse Practitioner - Bachelor degree in nursing and successful completion of a nurse practitioner program at the master level Physician Assistant - Graduate of accredited PA program
EXPERIENCE
Three or more years of professional experience as a nurse practitioner or physician assistant. Home Health experience required. Prior hospital experience with preference given to providers who have worked on general medical floors or in critical care or emergency department settings. Certificates, Licenses, Registrations Currently licensed in the State of Tennessee as a RN and ANCC certified as a nurse practitioner in a specialty area or licensed as a PA. BLS and ACLS Certification required.
