Position Summary:
The Advanced Practice Nurse (APN) for Organ Acquisition and Renal Transplant provides comprehensive, specialized care to renal transplant candidates and recipients across the continuum of transplantation. This role is responsible for the clinical management of renal transplant recipients, with a strong emphasis on immunosuppression management, rejection prevention, early identification and management of transplant related complications, and coordination of post-transplant care The APN collaborates closely with transplant surgeons, nephrologists, transplant coordinators, transplant pharmacists, and multidisciplinary team members to ensure evidence-based, patient-centered care. Responsibilities include assessment, diagnosis, and management of transplant-related conditions; monitoring graft function; adjusting immunosuppressive therapies; and managing acute and chronic rejection and post-transplant complications in accordance with established protocols and best practices. In addition, the APN plays a critical role in patient and family education, ensuring understanding of immunosuppressive regimens, adherence requirements, potential side effects, and long-term health maintenance. The role supports organ acquisition and transplant processes by contributing to candidate evaluation, post-transplant follow-up, quality improvement initiatives, and regulatory compliance. This position requires advanced clinical expertise, strong critical-thinking skills, and effective communication to optimize patient outcomes, promote graft longevity, and support the mission of the renal transplant program.
